To apply for any of the housing programs at FCHA, please complete our Housing Application Form. 
Please note that all fields marked with * must be completed for the form to be submitted. 

You will not be added to the waitlist until the following conditions have been met:

Sign all required paperwork at our WARCO office. All Household members over 18 must be present.

Provide original of the following documents for all household members (UPLOAD OR IN-PERSON)

• Identification (All household members over 18) 
• Income Verification (How you receive money)
• Check Stubs (Last 6)
• SS or SSI Award Letter
• K-Tap
• Investments
• Asset Verification (Where you hold money)
• Savings Last 6 Months Statements
• Checking Statements (Banks, Direct Express, PayPal, CashApp) Last 6 Months Statements
• Deeds for Properties
• Whole Life Insurance
• Proof of Age (Government Documents)
• Social Security Cards
• Food Stamp Verification
• Marriage License and/or Divorce Papers
• Four personal references not related to you or your family

Any Online Application, that has not been completed in our WARCO office, will be discarded after 15 Days.
